Abstract: Consumer and carer feedback at workshops in 2024 highlighted the importance of communication, home supports and discharge planning in shaping patient experience, identifying an opportunity to improve early recognition, targeted intervention and person-centred service design for older frail patients. The Acute Frailty &#xD;
Team (AFT), initiated through the Timely Emergency Care (TEC) &#xD;
improvement program, focuses on improving flow and patient experience &#xD;
by responding to the additional needs of older people living with frailty &#xD;
whose care is often influenced by factors beyond immediate presenting &#xD;
issues. The initative introduced screening and identified frail patients &#xD;
early in their ED journey and facilitated early intervention with senior &#xD;
clinicians.</description>

Abstract: The impelemntation of the National Clinical Trials Governance Framework issued by the Australian Commission on Safety &amp; Quality in Health Care gave us the opportunity to develop a process of collecting such patient feedback, so that we can better understand the needs of our clinical trial patients and optimise our clinical trial services accordingly. Research Support Services developed a survey, in consultation with the Patient Experiment team, and the Research Managers Group, that was open from August-September 2025 and will be conducted each year.</description>

Abstract: During consultation with the maternity General Practitioner (GP) advisory group, at the quarterly ‘Maternity GP and Shared Care &#xD;
Committee’ meeting, concerns were initially identified that the Hand-Held Pregnancy Record (HHR) was not being given to Shared Care patients, nor being used at the hospital by care providers. They also stated that results ordered by hospital care providers were also not being shared adequately. Community antenatal care &#xD;
providers were concerned that this affected their ability to provide quality and safe antenatal care. We had also received some feedback from patients that the shared care program was not feeling ‘safe’, as consultation notes and &#xD;
results were not being shared. They were therefore requesting to transfer in for hospital based models of care.</description>
