dc.description.abstractWe have made multiple person centred changes to the processes and procedures related to care of the Home Parenteral Nutrition (PN) clients over the past 2 years. Children requiring Home PN require frequent blood tests that need to be collected from their Central Venous Access Device (CVAD). This required an admission to the Same day unit every time. This same situation occurred for their frequently administered Iron and Carnitine medication infusions. Parents of children requiring Home PN identified that many health professionals were not familiar with their child’s long term CVAD line.They were often asked repetitive questions about the use and care of these lines.-
