dc.description.abstractOur aim was to improve discharge communication and to empower our patients to confidently manage their wound dressing regime. One suggestion was a wound care discharge plan to be handed out upon discharge, outlining specific wound care and dressing regime. A pilot team was created to initiate and implement this project. The new form was approved by Patient Experience Team based on consumer review/feedback.-
