In collaboration with Safer Care Victoria, South 5 committed to a year long collaboration to provide age-friendly care to patients (over the age of 65 years) on the subacute ward. Age-friendly healthcare systems provide a set of four evidence-based elements of high quality care, known as 4Ms - What Matters, Mind, Mobility and Medication.
