Abstract: Older people are more likely to be admitted to hospital than younger people, and more likely to experience harm, such as falls, physical deterioration, medication errors, infections or confusion. In collaboration with Safer Care Victoria, South 5 committed to a year long collaboration to provide age-friendly care to patients (over the age of 65 years) on the subacute ward. Age-friendly healthcare systems provide a set of four evidence-based elements of high quality care, known as 4Ms - What Matters, Mind, Mobility and Medication. Applied as a set, it is person-centred, proactive and holistic approach to care of the older person and reduces harm.Focusing on What Matters enables a deeper understanding of the older person in our care, allowing healthcare goals to be better aligned with that individual’s values.
