Background: Understanding the patient population of a health service informs decisionmaking and ultimately improves patient outcomes. A key data point for our hand therapy service is the diagnosis of each patient, representing the primary condition or injury for which we are providing therapy Aim: To record the diagnosis of each new hand therapy patient in an accurate, efficient and easily collatable and reportable format.
