Abstract: Clinical handovers aim to communicate relevant, accurate and up-to-date information about a patient's care to ensure patient safety. Monash Health uses the Clinical handover shift to shift bedside/close proximity procedure which defines the requirements and procedure for clinical staff to conduct handovers. Anecdotally, at Monash Health, handovers in the subacute clinical space vary across teams and differ with the acute Physiotherapy processes.
