These syndromes are mostly reversible if treated early. Tumour association has been reported in up to 60% of cases in the most recent case series with lung, breast, ovarian cancers and thymomas being the most commonly associated malignancies with these antibodies. We present here the first case of AMPAR encephalitis associated with medullary thyroid cancer. Our patient presented with cognitive dysfunction and behavioural changes over a period of 3 weeks, with a full recovery after starting immunotherapy, once the diagnosis of AMPAR limbic encephalitis was established. This case highlights the importance of early diagnosis and management of AMPAR encephalitis as these patients respond well to immunotherapy and can have an almost complete recovery.Copyright © 2018 BMJ Publishing Group Limited. | DOI: | http://monash.idm.oclc.org/login?url=http://dx.doi.org/10.1136/bcr-2018-225745 | Link to associated publication: | Click here for full text options | PubMed URL: | 30150348 [http://www.ncbi.nlm.nih.gov/pubmed/?term=30150348] | ISSN: | 1757-790X (electronic) | URI: | https://repository.monashhealth.org/monashhealthjspui/handle/1/37089 | Type: | Article | Subjects: | hysterectomy memory disorder nuclear magnetic resonance imaging paranoia positron emission tomography sciatica/dt [Drug Therapy] subclinical hyperthyroidism *thyroid medullary carcinoma/dt [Drug Therapy] *thyroid medullary carcinoma/rt [Radiotherapy] *thyroid medullary carcinoma/su [Surgery] thyroidectomy *AMPA receptor/ec [Endogenous Compound] antinuclear antibody/ec [Endogenous Compound] calcitonin/ec [Endogenous Compound] calcium escitalopram/dt [Drug Therapy] immunoglobulin/dt [Drug Therapy] immunoglobulin/iv [Intravenous Drug Administration] paracetamol/dt [Drug Therapy] risperidone/dt [Drug Therapy] thyroxine/dt [Drug Therapy] vitamin D *anti ampa receptor encephalitis/co [Complication] *anti ampa receptor encephalitis/di [Diagnosis] *anti ampa receptor encephalitis/dt [Drug Therapy] priority journal aged article case report cerebrospinal fluid analysis cervical lymphadenopathy clinical article computer assisted tomography confusion depression/dt [Drug Therapy] *encephalitis/co [Complication] *encephalitis/di [Diagnosis] *encephalitis/dt [Drug Therapy] female human memory disorder nuclear magnetic resonance imaging paranoia positron emission tomography priority journal sciatica / drug therapy subclinical hyperthyroidism *thyroid medullary carcinoma / *drug therapy / *radiotherapy / *surgery thyroidectomy Article clinical article computer assisted tomography confusion depression / drug therapy *encephalitis / *complication / *diagnosis / *drug therapy female human hysterectomy cervical lymphadenopathy cerebrospinal fluid analysis case report aged |
