The stroke rehabilitation clinical guide-A community rehabilitation focus.

Abstract

The Stroke Rehabilitation Clinical Guide (SRCG) was designed and trialed for Monash Health (MH) Kingston sub-acute services in 2013 as part of a Victorian Stroke Clinical Network (VSCN) funded initiative. The SRCG was designed to be transferable to any rehabilitation setting across VIC, inpatient or ambulatory. MH recognizes the importance of evidence based rehabilitation for stroke survivors and strives to provide the best possible care to these consumers. There has been significant recent work within the group of MH Community Rehabilitation (CR) programs to achieve evidence based practice where gaps exist in stroke care and the SRCG will allow for consolidation of this work and a streamlined, yet individualized, evidence based approach to this client population across CR. This project aims to review the SRCG inpatient engagement and progress; analyze current CR stroke survivor care and CR staff knowledge of the evidence; complete a gap analysis against the SRCG and; develop an action plan, in consultation with key stakeholders, to address identified gaps and promote future sustainability. The SRCG will be implemented on a trial basis within CR for 8 weeks from March 2015 and evaluated accordingly by June 30th 2015. As a result of this project, MH CR clinicians will have the resources and education to provide evidence based rehabilitation to stroke survivors in their care; the SRCG will be rolled out in CR in an adapted format and; stroke survivor care will be sustainably evaluated across CR to ensure currency of evidence based practice.
